Teacher Evaluation Survey

This tool is designed to collect data for the initial design phase of the Ethics Curriculum Project Modules. Data will be used to evaluate teacher satisfaction and student impact (if the learning objectives met the course content, if the objectives were met by the students and if the assessment activities in the module accurately measure student performance).


  1. The module was appropriate to the content area.

  2. The objectives were clear and appropriate for the learning activity.

  3. The assessments accurately reflected content and objectives.

  4. The media was appropriate for the instructional method.

  5. The time required to go through the instructional material was within class time constraints.

  6. The material was appropriate for my student population.

  7. The materials were interesting.

  8. The materials accurately reflected the content without bias.

  9. The materials were culturally appropriate.

  10. Adequate materials were supplied to facilitate ease of use.

  11. The module’s content was effective in getting the students to think and discuss ethical issues.

  12. The module interested the students.

  13. The module seemed to have relevance to the students.

  14. The module’s lessons helped foster student’s desires to understand alternative perspectives.

  15. The module was effective in getting students to understand their own perspective.

  16. Students found the activities stimulating.
